I have devised a house rule subsystem for characters to have a little less lethality, i.e. a chance to survive when reduced to 0 hp. My opinion is that in some situations, like one-shot games, less lethality is a good option while keeping all the other features of OSR play.
This rules are similar in intent to death saves from 5e but give the player more (risky) options, like acting while wounded.
